Press 1 for appointments:
Make sure to leave the following information in your voicemail.
- Patient's Name
- Date of Birth
- Insurance Name
- Insurance ID number
- Your phone number and relationship with the
Fill out the Appointment request form and submit.
Press 2 for billing issues:
Make sure to leave a detailed message in regards to
your issue, your name, patient's name, date of birth, date of Service in question and a number to contact you.
Press 3 for medication refills:
Make sure to leave the following information in your
- Patient's name
- Date of birth
- Medication name and strength
- Medication frequency (how many times a day)
- Pharmacy number
Please note, there is a 10$ charge for urgent refill requests (medications that need to filled on the same day of being requested or over weekends/holidays). Please plan ahead and give us 2-3 business days for refills. Stimulants can not be faxed or called in so please call ahead of time, these prescriptions are not filled over weekends.
Press 4 If you need to leave a message for Dr Amin:
Please make sure to state what is the urgency level of your call. Wether your call needs to be answered the same day or can wait till the next business day. Leave a short message with your name, patient's name, date of birth, clear reason for the call, a call back number.
Please know, no new medication are prescribed over a phone call, no medication dosages are changed over a phone call, You need to come in for a follow up appointment if you need your medication adjusted.
Phone calls are for urgent matters like medication side effects or other psychiatric emergencies.
If you need immediate assistance and can not wait for your doctor's call back, Please call 911 or go to the nearest ER right away.
